Employee Payroll Documents

All employees must submit the appropriate forms below in order to work and receive a paycheck. The documents are due at the first staff orientation.  Download these forms, Print, Fill Out, and Turn In to your camp director. 

•   Employment Eligibility Verification I-9 (Required for all camp staff)
•   IRS W-4 (Required for all paid camp staff)
•    Louisiana L-4 (Required for all paid camp staff)
• Staff Emergency Form ( Required for all camp staff)
• Counselor Employment Form (Required for all camp Counselors and CIT's)
• Staff Shirt Request Form (Required for all camp staff)
• Background Check Form (Required for all camp staff)
